The New York State Department of Health, Office of Continuing Care, licenses and conducts surveys and investigations of the quality of care and life for approximately 110,000 people residing in nursing homes in NY State. Nursing home residents are a diverse population including the elderly with chronic disabilities, infants with multiple impairments, and young adults suffering from traumatic brain injury or other physical disabilities. At least every 15 months, each nursing home in NY State has a standard survey conducted onsite by department staff to determine deficiencies. Whenever necessary, facilities also receive post-survey revisits to ensure that any deficiencies are corrected.

Information for emergency management purposes including locations, capacities, and emergency contact information are deemed sensitive and are available only to Health Department and Emergency Preparedness personnel. Individual data elements for each nursing home are not deemed sensitive, however, the availability of this information in a comprehensive dataset for all nursing homes may create an opportunity for misuse of information.

Each nursing home is uniquely identified by a single PFI value within the table [ID].
All facilities included in this data set are assumed operational as of the date of data extraction/receipt, however, no guarantee is made or implied regarding the status of any facility. Closed facilities are available by contacting GIS - Shared Solutions. Excluded from this data set are facilities classified as Long Term Home Health Care Programs and Certfied Home Health Agencies. These facilities are removed because they are home-based agencies and their corresponding geographical information only represents administrative offices. Note: Two conditions of closure may exist: physical and regulatory. Regulatory closure means the license to operate the facility is invalid and the facility is also physically closed. Physical closure means the facility is physically closed, but may have a valid continuing license that can erroneously lead a user to believe it's still in operation.
All locations were determined using street address information, street-level imagery, digital orthophotography, public web site information, and/or phone calls to facilities. However, ground verification was not performed and no guarantee is made or implied regarding the accuracy of facility locations.
This is an updated data set of Nursing Homes. Data were extracted from the NYS Health Facilities Information System (HFIS) maintained by the NYS Department of Health. The facilities contained in this data set were extracted from a master database for use in DOH critical infrastructure applications, and other disaster preparedness and response applications.
